  • 당뇨병은 전 세계적으로 가장 흔한 만성 질환 중 하나로, 50세 이상 성인에서 실명의 주요한 원인이다. 당뇨병이 있는 환자는 백내장 발생 위험이 높고, 질환의 진행 속도도 빠르며 당뇨가 없는 환자보다 더 이른 나이에 수술을 받게 되는 경향이 있다. 본 종설에서는 당뇨환자의 백내장 수술 전 평가부터 수술 후 관리까지 주의해야 할 사항을 정리하고자 한다. 당뇨가 있는 환자는 굴절상태가 불안정하며 안구건조증, 각막상피결손, 반복적인 각막미란의 발생 위험이 높다. 또한 각막내피세포 밀도가 낮고 동공이 작아 수술 시 합병증이 발생하기 쉽다. 수술 후에는 위수정체 낭포황반부종, 후낭혼탁, 안내염, 당뇨망막병증의 악화, 신생혈관녹내장 등의 합병증이 발생할 수 있다. 특히 당뇨 유병기간이 길거나 혈당 조절이 불량한 경우 합병증 발생 위험이 더욱 높아지므로 정기적인 안과 추적 검사가 필요하다. 또한 적절한 시기에 항혈관내피성장인자 주사와 같은 보조 치료를 병행함으로써 합병증 발생을 최소화하는 것이 중요하다.

  • Diabetes mellitus is one of the most common chronic diseases worldwide and is a leading cause of blindness in patients over the age of 50 years. Patients with diabetes have an elevated risk of developing cataracts compared to individuals without diabetes; furthermore, cataracts also tend to progress more rapidly in this population, leading to the need for surgery at a younger age. This review aims to summarize the key considerations in the management of cataract surgery in patients with diabetes, from preoperative evaluation to postoperative care. Patients with diabetes often present with unstable refractive status, dry eye disease, corneal epithelial defects, and recurrent corneal erosions. They also tend to have reduced corneal endothelial cell density and small pupils, both of which increase the risk of intraoperative complications. Postoperatively, these patients are at risk of developing pseudophakic cystoid macular edema, posterior capsular opacification, endophthalmitis, progression of diabetic retinopathy, and neovascular glaucoma. Patients with long-standing or poorly controlled diabetes face a higher likelihood of postoperative complications, highlighting the importance of regular ophthalmic follow-up examinations. Furthermore, adjunctive treatments such as timely intravitreal injections of anti-vascular endothelial growth factor agents may reduce the risk of vision-threatening complications following cataract surgery.

당뇨병은 전 세계적으로 매우 흔한 만성 대사성 질환으로, 2025년에는 환자 수가 약 5억 7천만 명에 이를 것으로 추정된다[1]. 전 세계적인 백내장 유병률은 약 17.2%이며[2] 50세 이상 환자에서 실명의 주요한 원인으로 보고된다[3]. 당뇨환자는 비 당뇨환자보다 백내장이 진행될 확률이 약 5배 높으며, 더 어린 나이에 수술을 받게 된다[4]. 백내장 발생의 상대위험도는 당뇨병의 지속 기간, 고혈당의 중증도, 연령에 따라 증가하게 된다[5].
당뇨환자에서 백내장 발생의 병태생리는 수정체 내 당화산물의 축적, 삼투압의 변화, 산화스트레스의 증가, 폴리올 경로(polyol pathway)의 활성증가 등이 거론된다. 당뇨병성 백내장의 양상은 일반적으로는 기타 노년백내장과 비슷한 형태로 나타나지만, 더 일찍 발병하고 빠르게 진행하는 경향을 보이며, 연소 당뇨환자에서는 전낭과 후낭에 공포(vacuole)와 함께 눈송이백내장(snow flake cataract) 형태를 보일 수 있다[6].
백내장 수술 시기는 백내장의 정도와 당뇨망막병증 진행 정도에 따라 달라질 수 있지만 환자의 시력 저하가 백내장 때문인지 당뇨망막병증 때문인지부터 구별해야 한다. 이를 위해서는 철저한 안저검사, 빛간섭단층촬영검사를 이용한 당뇨황반부종 여부를 확인해야 한다. 백내장이 심하게 진행하여 망막출혈이나 신생혈관을 관찰하기 어려워지기 전에, 그리고, 범망막광응고치료를 위한 아르곤레이저가 효과적으로 수정체를 통과할 수 있도록 조기 수술을 하는 것도 고려해야 한다. 급격한 혈당조절은 오히려 수술 후 당뇨망막병증과 당뇨황반부종의 진행 위험성을 높이기 때문에 내과 주치의 관리하에 천천히 조절해야 한다[7].
고혈압, 심혈관 질환 병력, 항응고제 및 항혈소판제 복용 여부, 투석 여부 등 동반 질환도 평가해야 한다. 국소점안마취와 투명각막절개를 시행할 경우 항응고제를 중단하지 않아도 되지만 복잡한 백내장 수술이 예상될 때는 주치의와 상의하여 항응고제의 일시적 중단이 필요할 수도 있다.
당뇨황반부종, 당뇨망막병증, 신생혈관녹내장은 백내장 수술을 시행하기 전에 반드시 치료해야 한다. 특히, 황반부종은 시력 예후에 매우 중요한 인자이므로 국소레이저광응고술 또는 항혈관내피성장인자(anti-vascular endothelial growth factor)나 코르티코스테로이드의 유리체강내 주사로 치료해야 한다. 위수정체 낭포황반부종을 예방하기 위해서는 수술 전 비스테로이드성 항염증제를 사용하는 것이 도움이 된다[8]. 증식당뇨망막병증은 범망막광응고술 또는 항혈관내피성장인자 주사로 최소한 수술 1–2주 전에 치료를 시작하고, 수술 후에도 반복 주사가 필요하다. 신생혈관녹내장은 백내장 수술 전·중·후에 발생하고 있는지를 지속적으로 확인해야 한다. 안압이 조절되지 않는 신생혈관녹내장 환자에서는 백내장과 함께 녹내장 병합수술을 시행할 수 있다. 그러나 침습적인 녹내장 수술을 백내장 수술과 함께 시행하면 전방출혈, 창상누출, 얕은 전방, 저안압과 같은 합병증 발생이 많고, 시력회복이 느리며, 백내장 수술 후 염증으로 인해 여과포 형성에 영향을 주어 녹내장 수술 결과에 좋지 않은 영향을 미칠 수 있기에 병합술보다 단계적 접근이 추천된다[9].
유리체강내 주사를 여러 번 시행 받은 환자는 수술 중 섬모체소대 약화나 후낭파열 위험이 증가하므로 이에 대한 대비가 필요하다[10,11]. 또한, 과거 유리체절제술을 받은 경우에는 후방의 압력 변화가 심해 전방의 깊이가 급격하게 변할 수 있으므로 각별한 주의가 필요하다. 2010년대 이후 도입된 초음파수정체유화기는 전방 깊이와 압력을 비교적 일정하게 유지할 수 있어 이러한 합병증의 위험을 다소 줄일 수 있게 되었다[12]. 따라서 수술 전에는 수정체낭내팽창고리(capsular tension ring) 등을 미리 준비하고, 인공수정체를 수정체낭 내에 고정할 수 없는 경우에 대비한 렌즈 도수 계산도 수술 전 준비 과정에 포함되어야 한다.
당뇨환자에서는 혈당 수치 변화에 따라 굴절 변화가 발생할 수 있다(Table 1). 고혈당 상태가 되면 –0.5 diopter (D)에서 최대 –3.75 D까지 근시성 변화가 발생할 수 있다. 급격히 혈당조절을 하게 되면 1–2주 내에 정상화된다. 고혈당 상태에서 근시가 발생하는 이유는 수정체 내의 과잉 포도당이 알도스환원 효소에 의해 소르비톨로 전환되면서 삼투압이 올라가 수정체의 굴절력이 증가하기 때문이다[13]. 수정체 두께의 변화가 객관적으로 확인되지 않더라도, 수정체 내의 굴절률 변화는 나타나게 된다[14]. 고혈당에서 저혈당으로 급격히 변하면 포도당은 수정체에서 빠르게 빠져나갈 수 있지만 소르비톨은 빠져나가지 못한다. 당뇨환자에서 혈당 조절이 불안정한 경우 이러한 삼투압 차이가 방수의 유입과 수정체 팽창을 초래하여 당뇨병성 백내장을 일으키게 된다. 제1형 당뇨환자가 정상 대조군보다 수정체가 더 두껍고 전방 깊이가 얕으며[15], 당뇨안에서는 각막신경밀도가 감소해 각막지각이 떨어져 지속적인 각막상피결손, 안구건조증, 마이봄샘기능이상증 등을 유발하게 되고, 이는 난시를 비롯한 각막굴절력 검사에 오차를 유발할 수 있다[16]. 또한, 중심각막두께가 증가하여 각막 굴절력에 영향을 미칠 수 있으며, 이는 각막내피세포의 기능 저하로 인한 것으로 추정된다[17]. 혈당수치에 따라 각막지형도가 변화할 수 있음이 보고되었다[18].
임상적으로 안축장은 크게 변화가 없지만, 당뇨황반부종으로 망막이 두꺼워진 경우 접촉식 초음파안축장 측정기보다는 눈계측검사기기로 안축장을 측정하는 것이 덜 영향을 받는다[19].
현재까지 당뇨병이나 혈당 변화가 생체계측의 최종 결과에 직접적인 영향을 준다는 근거는 없다. 그러나 당뇨환자에서 생체계측의 신뢰도가 정상인에 비해 낮을 수 있으며, 환자의 현굴절력이 당뇨 수치로 인해 왜곡되었을 수 있음을 인지하고 인공수정체 도수 선택 시 고려되어야 할 것이다.
안구건조증 및 상피 치유 지연
당뇨병은 안구건조증의 주요 위험 인자 중 하나로 당뇨환자의 최대 54%가 안구건조증을 호소한다[20]. 당뇨환자에서 안구건조증 증상은 특히 신경병증, 즉 각막지각저하와 관련이 있다. 이들의 각막신경망은 밀도가 낮고, 신경이 더 꼬불거리며 불규칙하다[21]. 또한 결막인상세포검사(conjunctival impression cytology)에서도 편평상피화(squamous metaplasia) 정도가 높고, 술잔세포(goblet cell) 밀도는 낮다. 장기적인 고혈당은 눈물샘 기능장애도 초래한다[21].
당뇨환자의 각막상피기저막과 상피-기질 간 상호작용의 손상은[22] 상피 결손, 재발성 각막미란, 점상 각막병증, 감각 저하, 상피화 지연, 상처 치유 이상, 손상에 대한 취약성 증가 및 각막궤양의 위험을 높인다[23]. 또한, 당뇨환자는 성장인자 부족, 비정상적인 혈관 신생, 대식세포 기능 이상, 콜라겐 축적 이상, 그리고 각질세포(keratinocyte) 및 섬유아세포의 이동 및 증식 이상으로 적절한 각막 치유가 지연된다[24]. 이러한 특징은 수술 중 또는 수술 후 발생한 각막 찰과상의 치유 지연과 반복적 각막짓무름으로 이어질 수 있다[25]. 심한 경우는 백내장 수술 중 상피기저막이상으로 수포성 상피 박리(bullous epithelial detachment)가 발생한 사례도 있다[26].
따라서 백내장 수술 시 개검기를 삽입할 때부터 각막에 상처를 주지 않도록 해야 하며 각막에 물을 뿌릴 때도 강하게 쏘지 않게 각별한 주의를 기울여야 한다. 백내장 수술 후 안구건조증은 수술로 인한 기계적 외상, 현미경 조명 노출, 사용하는 안약 성분 및 보존제, 눈꺼풀염 등과 관련이 있지만[27] 당뇨환자에서는 각막지각저하로 인한 눈물 분비량 감소 및 신경영양효과의 감소가 더 쉽게 안구건조증을 악화시키거나 새롭게 안구건조증을 유발할 수 있다[28].
각막내피세포 손상
당뇨환자는 동일 연령대의 정상인에 비해 각막내피세포 밀도가 낮다[29]. 이는 당뇨유병기간 및 당화혈색소 수치와 역상관관계를 보이며[30,31] 제1형 당뇨가 제2형 당뇨보다 더 낮은 밀도를 보인다[32]. 또한, 당뇨환자에서는 육각형 내피세포 비율 감소, 다형성(polymegathism) 증가, 중심각막두께 증가가 관찰된다[33]. 이러한 내피세포는 외상에 더 민감해지고, 회복능력도 떨어진다. 이는 내피세포의 Na⁺–K⁺–ATPase 활성 저하와 방수에서 증가한 포도당 농도가 각막기질의 산증(acidosis)과 회복능을 저하시키기 때문이다[34].
당뇨환자에서는 수술 중 축동이 더 자주 발생하며, 이로 인해 초음파수정체유화술 시 낭내가 아닌 전방에서 실행되어 각막내피세포에 손상이 가중될 수 있다[35]. 당뇨환자는 각막기저하 신경망(sub basal nerve plexus) 밀도가 낮고, 백내장 수술 시 가해진 손상이 당뇨병성 각막병증(diabetic keratopathy)을 유발하여 각막부종이 더 자주 발생한다[36].
결막염 및 안검염
당뇨환자는 정상인에 비해 결막에 황색포도알균(Staphylococcus aureus), 장알균(Enterococcus), 일부 사슬알균(Streptococcus), 클레브시엘라(Klebsiella) 등의 세균이 더 많이 상주한다[37]. 특히, 결막 내 세균 집락 형성은 당뇨망막병증의 중증도와 관련이 있다고 알려진다[37]. 당뇨환자는 급성 감염성 결막염에 걸릴 위험이 정상인에 비해 1.24배 더 높다고 하였는데[38], 이는 고혈당 환경이 세균 증식을 촉진하고, 정상적인 결막 세균총의 균형을 무너뜨릴 수 있기 때문이다[38].
당뇨환자에서는 눈꺼풀 가장자리의 분비물, 세균 감염, 마이봄샘의 기능 이상 등이 호발하므로, 백내장 수술 전·후 철저한 눈꺼풀과 결막 소독이 필요하다.
안내염
당뇨환자는 폐렴, 요로감염, 수술 상처 감염, 폐혈증 등 전신 감염에 취약하다. 고혈당은 호중구 기능 저하, 항산화 시스템 억제, 체액성 면역 저하 등 면역 기능장애를 일으킨다[39]. 예를 들어, 칸디다알비칸스(Candida albicans)는 당뇨환자의 세포에 더 잘 달라붙고, 고혈당 환경에서는 더 강한 병원성을 지녀서 감염을 쉽게 유발하게 된다[40].
한 대규모 역학연구에 따르면, 당뇨환자는 백내장 수술 후 안내염 발생 위험이 비당뇨환자에 비해 2.92배 높으며 예후도 나쁘다고 하였다[41]. 당뇨환자는 각막절개창의 치유가 늦어져서 상처가 잘 벌어지고 회복이 느리며 결막에 세균 집락 형성률도 더 높기 때문이다. Endophthalmitis Vitrectomy Study Group 연구에 따르면, 안내염 발생 후 시력이 20/40 이상으로 회복된 비율이 정상인은 55%였던 반면 당뇨환자는 39%였다[42].
당뇨환자는 자주 동공이 작고, 대광반사가 느리며 산동제에 잘 반응하지 않는다[43]. 이는 홍채의 색소상피 및 홍채조임근(iris sphincter muscle)과 홍채산대근(iris dilator muscle)에 글리코겐이 침착되어 동공대광반사가 둔화되어 있으며, 당뇨자율신경병증(autonomic neuropathy)에 의해 산동이 잘 안되고 시간도 오래 걸리기 때문이다. 홍채조임근은 주로 부교감신경계의 영향을 받기 때문에 상대적으로 당뇨병에 의한 영향이 적지만, 당뇨환자에서는 홍채산대근의 교감신경계 손상이 불충분한 산동의 주요 원인으로 작용한다[44]. 따라서 항콜린성 산동제보다는 10% 페닐에프린[45], 수술 중 전방 내 에피네프린(최대 1:2,500 희석액) [46], 무보존제 1% 리도카과[47] 같이 홍채산대근에 직접 작용 가능한 교감신경 흥분제를 사용하는 것이 추천된다. 교감신경 손상은 당뇨병 지속 기간 및 전신 자율신경병증의 이상과 밀접하게 관련되어 있다[48]. 망막에 범망막광응고레이저술을 받은 경우 그렇지 않은 경우보다 산동이 잘 안된다[49]. 또한, 홍채혈관의 투과성이 증가되어 있어 수술 중 홍채를 건드리면 정상보다 더 쉽게 축동되므로 수술 전 충분한 산동을 준비해야 한다.
수술 중 응집력이 높은 고분자량 점탄물질(2.3% sodium hyaluronate)로 동공을 키울 수도 있다. 이는 전방을 깊게 하고, 수정체전낭을 편평하게 하여 수정체낭원형절개를 쉽게 할 수 있게 도와주지만, 과도한 양은 섬모체소대에 긴장을 초래할 수 있고 정상인에 비해 당뇨환자는 생각보다 동공이 잘 키워지지 않는다.
약제 사용에도 불구하고 동공이 커지지 않으면 홍채확장고리나 홍채갈고리를 이용하게 된다. 국내에서 두 종류의 홍채확장고리를 사용할 수 있다. 티타늄 합금으로 만들어진 XpandNT (Diamatrix Ltd.)는 동공 지름을 6.7 mm로 확장시킬 수 있으며, 고압증기멸균으로 소독해야 한다. 5-폴리프로필렌으로 만들어진 Malyugin ring (MicroSurgical Technology)은 동공 지름을 6.25 mm로 확장시킬 수 있으며 ethylene oxide 가스 소독을 시행한다. 홍채확장고리는 눈이 작고 깊은 경우 유용하며, 빠르고 쉽게 넣고 뺄 수 있다. 하지만 홍채에만 걸 수 있으며 동공 크기 조절이 불가능하다. 홍채 갈고리는 삽입 위치와 개수, 견인력, 홍채 지름을 다양하게 조절할 수 있으며, 홍채뿐만 아니라 원형절개된 수정체낭에도 걸 수 있는 장점이 있다. 기구를 이용해 홍채를 과도하게 조작할 경우 색소 분산, 출혈, 동공 변형 및 위축, 수술 후 염증 증가 등이 발생할 수 있으므로 조심스럽게 삽입 및 제거를 해야 한다(Table 2). 특히, 당뇨환자에서는 홍채를 자극하게 되면 홍채혈관으로부터 누출이 쉽게 일어나고, 추후 당뇨황반부종의 발생을 일으킬 수 있다. 작은 동공 크기로 인해 수술 시간이 길어지게 되면 당뇨환자에서는 광독성망막병증(photic retinopathy)이 더 흔하게 나타나므로 주의가 필요하다[50].
당뇨환자는 백내장 수술 후 동공 크기가 감소되는 경향성이 있는데, 이는 수술 후 염증 반응일 것으로 추정하며[51] 대개 수술 후 3달이면 회복된다.
당뇨환자에서 백내장 수술이 당뇨망막병증에 미치는 영향은 잘 알려진 사실이다. 총 7,287명의 환자를 포함한 15편의 연구의 메타분석을[52] 확인해 보면 백내장 수술 후 당뇨망막병증 발생률은 대조군에 비해 증가(상대위험도 1.38)하며, 당뇨망막병증의 진행은 수술 종류나 연구 설계와 관계없이 유의하게 더 높았다(상대위험도 1.46). 또한, 시력에 지장을 주는 당뇨황반부종과 증식당뇨망막병증으로의 진행 역시 유의하게 증가하였다(상대위험도 1.84).
지금은 시행되지 않지만, 과거 주된 백내장 수술이었던 백내장낭외적출술 환자의 70%에서 당뇨망막병증이 진행되었고[53] 초음파수정체유화술 후에는 20%에서 진행하였다[54]. 당뇨망막병증의 악화는 수술 전 당뇨망막병증의 단계, 당뇨병 유병 기간, 그리고 혈당 조절 상태에 따라 달라진다[55]. 따라서 경도의 백내장을 가진 증식당뇨망막병증 환자의 경우, 범망막광응고술 후 최소 1년 이상 망막상태가 안정된 후 수술을 추천하며, 중등도 이상의 백내장을 가진 경우 수술 후 범망막광응고술을 가능하면 빨리 시행하는 것이 좋다[56]. 또한, 항혈관내피성장인자 주사는 유의하게 당뇨망막병증의 진행을 예방할 수 있기 때문에 약제의 유리체강내 주사를 고려해야 한다[57,58].
이러한 당뇨망막병증의 진행은 전방 내 술기조작과 관련된 물리적 외상으로 인해 염증 반응이 유발되기 때문으로 추정된다. 포도막 조직에서 아라키돈산이 방출되면, 이는 지질산화효소 경로를 통한 류코트리엔이나 사이클로옥시제나제 경로를 통한 프로스타글란딘 생성으로 이어진다. 이러한 염증 매개물질이 후극부로 확산되어 당뇨망막병증의 진행을 초래하고, 혈관방수장벽(blood-aqueous barrier)의 파괴와 황반부종으로 이어진다[55].
낭포황반부종은 백내장 수술 후 시력 저하의 가장 흔한 원인이며[59], 발생빈도는 1%에서 3%로 보고되고 있다[60]. 당뇨환자에서 위수정체 낭포황반부종과 당뇨황반부종을 구분하는 것은 어려우며 형광안저혈관조영으로 전형전인 꽃 모양 누출(petaloid leakage)을 확인하면 된다. 그러나 두 질환의 치료가 유사하기 때문에 침습적인 검사를 시행하지 않고 경험적인 치료를 시행할 수도 있다. 당뇨병이 위수정체 낭포황반부종 발생의 위험인자임은 잘 알려진 사실이다[59]. 당뇨망막병증이 없는 환자에서 백내장 수술 후 낭포황반부종의 상대위험도는 1.8인데 비해 비증식당뇨망막병증 환자에서는 6.23, 증식당뇨망막병증 환자에서는 10.34로 급격히 상승한다[61]. 당뇨병의 유병 기간, 중증도뿐 아니라 수정체의 경화 정도와 당화혈색소 수치도 위수정체 낭포황반부종의 위험인자로 확인되었다[62]. 이러한 결과는 환자의 당뇨망막병증의 단계에 따라 적절한 예방이 필요함을 시사한다.
프레드니솔론 아세테이트 1.0%와 비스테로이드 항염증 안약을 병용해서 사용하면 스테로이드 단독 사용군보다 위수정체 낭포황반부종이 유의하게 낮게 발생하므로 당뇨환자에서 두 약제의 병용 사용이 권고된다[63]. 위수정체 낭포황반부종의 고위험을 갖고 있는 환자에게 백내장 수술 종료 시 결막하 트리암시놀론 아세토나이드 40 mg을 결막하 주사하면 황반 두께를 감소시킬 수 있지만 술 후 안압 상승 등의 부작용도 있어 주의를 요한다[64].
백내장 수술 후 3–6개월 사이에 당뇨황반부종의 발생 위험이 높으며[59], 수술 후 1년 이내에 발생할 위험은 당뇨망막병증의 진행 단계와 밀접하게 관련되어 있다. 한 연구에 의하면 당뇨망막병증이 없는 경우 발생률은 1.0%, 경도의 비증식당뇨망막병증에서는 5.4%, 중등도 비증식당뇨망막병증에서는 10.0%, 증식당뇨망막병증에서는 4.9%로 보고되었다[65]. 당뇨황반부종의 일차 치료로는 항혈관내피성장인자 약물이 권장되지만[66-68], 유리체강내 스테로이드 주사 또한 시력 호전 및 황반두께 감소에 유사한 효과를 보인다. 하지만 수술 후 6개월 뒤 지속적인 황반두께의 감소 효과는 항혈관내피성장인자 약물이 조금 더 우세하였다[69].
제1형 당뇨 환자에서 집중적인 당뇨 치료는 장기적으로 안과 수술을 받을 위험을 낮춘다는 근거가 있지만[70], 반대로 수술 전 3개월 이내에 급격한 혈당 교정을 시행한 환자에서 백내장 수술 후 당뇨망막병증 및 당뇨황반부종이 증가하기도 한다[7]. 비록 백내장 수술이 합병증 없이 진행되더라도, 수술 후 당뇨망막병증과 당뇨황반부종이 악화될 수 있으며, 특히 당화혈색소 수치가 높을수록 망막 합병증이 심화되고, 황반부종의 위험도 증가한다. 따라서 정기적인 안과 검진을 통해 면밀한 추적 관찰이 필요하다[61].
당뇨환자는 백내장 수술 후 후낭혼탁의 발생 위험이 높다[71]. 수술 후 초기에는 당뇨환자와 정상 환자 간에 차이가 없으나 수술 후 6개월 이후부터 후낭혼탁의 발생률에 차이가 발생한다[72]. 전신 상태, 당뇨병 치료 방법, 당뇨병 유병 기간과 후낭혼탁 정도 간에 연관성이 없다는 보고도 있지만[71], 당뇨병 유병 기간이 길수록 후낭혼탁 발생 위험이 높다는 보고도 있다[72].
인공수정체의 모서리가 뚜렷하게 각이 지면 수정체상피세포 증식을 줄이고 후낭혼탁 발생을 감소시킬 수 있다[73]. 친수성 인공수정체는 당뇨환자 방수의 인산염과 상호작용하여 석회화(calcification) 위험을 증가시키므로[74] 소수성 재질의 인공수정체를 삽입해야 한다. 또한 향후 평면부유리체절제술이 필요할 수 있으므로 실리콘 재질의 인공수정체도 피해야 한다[75]. 홍채집게형(iris-claw) 인공수정체는 홍채 신생혈관 위험을 높일 수 있고 홍채를 자극하여 황반부종을 유발할 수 있으므로 피해야 하며, 활동성 당뇨망막병증 환자에서는 다초점 인공수정체의 사용을 피하는 것이 좋다. 이는 향후 유리체절제술을 어렵게 하고 황반병증으로 인한 대비감도 저하를 악화시켜 시각적 어려움을 증가시킬 수 있기 때문이다[76].
당뇨가 잘 조절되고 당뇨망막병증이 없다면 다초점 인공수정체나 심도 확장(extended depth of focus) 렌즈를 고려할 수 있다. 그러나 환자가 향후 당뇨망막병증이 발생할 경우 단초점 렌즈에 비해 시력의 질이 떨어질 수 있음을 충분히 설명해야 한다.
백내장 수술 후 신생혈관녹내장의 발생과 안구 출혈은 심각한 결과를 초래할 수 있기 때문에 수술 전 홍채나 전방각에 신생혈관이 존재하는지 미리 철저한 검사를 해야 하며, 사전에 반드시 치료를 시행해야 한다.
당뇨망막병증에서 수정체의 제거는 녹내장에 대한 방어막을 없애는 것과 유사하다[77]. 당뇨망막병증으로 유리체절제술을 시행하면서 수정체를 제거한 경우는 수정체를 그대로 둔 경우보다 홍채신생혈관이 3배 더 발생한다[78].
그 이유로는 수정체낭의 제4형 콜라겐은 큰 분자의 이동을 제한하는 확산장벽으로 작용하여 혈관내피성장인자와 같은 큰 분자가 안구의 후부에서 전안부로 이동하는 것을 제한할 수 있는데, 수정체낭과 함께 수정체가 제거되면 이러한 인자의 이동이 수월하게 된다[79]. 유사하게 당뇨망막병증이 없는 당뇨환자에서 야그레이저를 이용한 후낭절개술 후 갑자기 신생혈관녹내장이 발생하는 증례도 여러 차례 보고된 바 있다[80,81]. 또한 수정체낭의 제4형 콜라겐의 구성 요소 중 일부는 내인성 신생혈관형성 억제 기능을 갖는다[82]. 전체 수정체 단백질 중 약 40%를 차지하는 알파 크리스탈린(α-crystallin)은 척추동물의 수정체에서 변성된 단백질에 결합하여 단백의 응집을 억제하고 수정체의 투명성을 유지한다. 특히, 알파-B 크리스탈린(αB-crystallin)은 혈관내피성장인자에 의해 유도된 인간망막미세혈관 내피세포의 증식을 억제하며, 혈관내피성장인자에 결합해 이를 안정화시켜 혈관신생을 조절하는 역할을 한다. 수정체제거술로 알파-B 크리스탈린이 제거되면 혈관신생조절을 할 수 없게 된다. 또 다른 원인으로는 백내장 수술 시 일어나는 안압의 급격한 변동과 안구혈류의 장애는 안구의 허혈성 변화를 악화시켜 신생혈관의 생성을 자극하는 것으로 추정되고 있다[83].
항혈관내피성장인자의 사용은 당뇨환자에게 유용하게 사용된다. 총 196명을 대상으로 한 메타분석에서 수술 종료 후 시행한 항혈관내피성장인자의 유리체강내 주사는 당뇨망막병증의 진행을 유의하게 억제시키며(상대위험도 0.37) 최대교정시력을 개선시킬 수 있었다[57]. 전향적 무작위 연구에서는 백내장 수술과 1.25 mg 베바시주맙 주사의 병합 치료가 당뇨망막병증 진행 예방에 미치는 역할을 비교하였는데, 주사군에서는 11%에서 당뇨망막병증이 진행한 반면 대조군에서는 45%에서 진행하였다[58]. 따라서 당뇨망막병증의 진행 위험이 높은 환자에서는 백내장 수술을 종료하면서 항혈관내피성장인자의 유리체강내 주사를 고려해야 한다.
항혈관내피성장인자 약물의 당뇨황반부종에 대한 효과는 아직 논란이 있다. 황반부종예방연구(the prevention of macular edema study)에서는 백내장 수술과 동시에 시행한 1.25 mg 베바시주맙 유리체 내 주사가 일반적인 당뇨환자에서 황반두께에 유의한 영향을 미치지 않는다고 하였지만[64], 다른 여러 연구에서는 당뇨황반부종 환자가 백내장 수술을 받을 때, 항혈관내피성장인자 주사 치료를 병용한 경우 황반두께 감소 효과가 보고되었고[66], 생리식염수를 투여한 대조군에 비해 황반부종악화를 예방할 수 있음을 보고하였다[67]. 일반적으로 유리체강내 주사는 수술 종료 시 평면부로 투여하지만, 일부에서는 수술 시작 시 투여해도 된다[68].
항혈관내피성장인자는 안구 내 혈관내피성장인자 농도를 직접적으로 감소시키기 때문에 망막, 홍채, 전방각의 신생혈관을 빠르고 효과적으로 치료할 수 있다[84]. 백내장 수술 전, 수술 중, 수술 후 모두 시행할 수 있으므로 신생혈관이 발견된 즉시 주사를 시행하여 합병증을 예방해야 한다.
초음파수정체유화술기와 수술 장비의 발달로, 당뇨 여부와 상관없이 백내장 수술 후 우수한 최대교정시력 결과를 얻을 수 있게 되었다. 그러나 당뇨환자의 백내장 수술은 수술 전 평가부터 수술 후 관리까지 여러 가지 어려움과 잠재적 합병증이 존재하므로 이를 충분히 고려해야 한다(Table 3). 수술 전에는 면밀하게 당뇨 조절 상태, 백내장 정도, 산동 정도, 당뇨망막병증 정도, 신생혈관 여부를 평가하고, 수술 시기와 인공수정체 선택을 신중히 고려해야 하며, 필요시 적절한 사전 치료를 완료한 후 수술을 시행해야 한다. 각막내피세포가 외상에 취약한 상태에서 동공이 작은 경우가 많아 백내장 술기를 기술적으로 더 어렵게 만들어 각막부종이 흔히 발생한다. 수술 후 안구건조증, 각막상피 치유 지연, 감염, 위수정체 낭포황반부종, 후낭혼탁, 당뇨망막병증 등의 발생 위험도 증가한다. 특히, 중증 비증식당뇨망막병증, 증식당뇨망막병증, 당뇨황반부종, 홍채신생혈관이 있는 경우 유리체강내 항혈관내피성장인자 주사와 같은 보조적 치료가 백내장 수술과 관련된 악화를 억제하는 데 도움이 될 수 있다.
당뇨환자의 백내장 수술에서 당뇨망막병증 및 당뇨황반부종의 악화, 낭포황반부종, 감염, 안구건조증이 시력 예후를 좌우한다. 따라서 수술 전 위험도 평가 및 적절한 술 전 처치, 수술 중 작은 동공의 관리 및 적절한 인공수정체 선택, 수술 후 모니터링 및 보조 치료 등 단계별 관리 전략을 통해 합병증을 최소화할 수 있다.
