    백내장 수술 시 연속초점 인공수정체에서 난시 인공수정체와 윤부이완절개술의 시력 효과를 비교하였다.
  • 방법
    연속초점 인공수정체를 이용하여 백내장 수술을 받은 환자들 중 난시 인공수정체를 사용하여 수술 시행한 60명의 환자군과 윤부이완절개술을 시행한 60명의 환자군을 대상으로 수술 후 임상 경과를 비교하였다. 각막난시가 1.0 diopters (D)를 초과할 때 난시 인공수정체를 삽입하였고(1군), 각막난시가 1.0 D 이하인 경우 윤부이완절개술을 시행하였다(2군). 각 군에서 수술 전과 수술 3개월 후 각막난시, 굴절난시, 굴절률, 나안시력, 교정시력에 대하여 분석하였다.
  • 결과
    수술 전과 수술 3개월 후의 각막난시와 전체 각막난시를 비교한 결과, 1군에서 통계적으로 유의한 변화는 없었으나(P>0.05), 2군에서는 수술 후(0.60±0.13 D, 0.61±0.28 D)가 수술 전(0.81±0.36 D, 1.25±0.32 D)에 비해 각각 유의하게 감소하였다. 굴절난시는 1군과 2군 모두 수술 전(2.05±0.64 D, 1.18±0.41 D)보다 수술 3개월 후(0.56±0.32 and 0.62±0.26 D)에 유의하게 감소하였으나, 1군의 감소 폭이 2군보다 유의하게 컸다(P<0.05). 수술 3개월 후 굴절난시는 두 군 모두 통계적인 차이는 없었다(P>0.05). 1군과 2군에서 모두 수술 3개월 후 나안시력과 교정시력이 수술 전 나안시력과 교정시력보다 유의하게 개선되었으나(P<0.05), 두 군 간의 차이는 없었다(P>0.05).
  • 결론
    경도의 난시에는 연속초점 인공수정체와 윤부이완절개술 병행이 효과적일 수 있으며, 중등도 이상의 난시에서는 연속초점 난시 인공수정체를 선택하는 것이 환자의 시력 개선에 도움이 된다.
  • Purpose
    This study aimed to compare visual and refractive outcomes after toric intraocular lens (IOL) implantation and limbal relaxing incision (LRI) in eyes receiving extended-depth-of-focus (EDOF) IOLs.
  • Methods
    Clinical outcomes were compared between 60 patients who underwent EDOF toric IOL implantation and 60 patients who underwent EDOF IOL implantation combined with LRI. A toric IOL was implanted when corneal astigmatism exceeded 1.0 diopters (D) (group 1), whereas LRI was performed when corneal astigmatism was 1.0 D or less (group 2). Corneal astigmatism, refractive astigmatism, refraction, uncorrected visual acuity, and corrected visual acuity (logMAR) were analyzed in each group before surgery and 3 months postoperatively.
  • Results
    Corneal and total corneal astigmatism did not differ significantly between preoperative and postoperative measurements in group 1 (P>0.05). In group 2, postoperative corneal and total corneal astigmatism (0.60±0.13 and 0.61±0.28 D, respectively) were significantly lower than the corresponding preoperative values (0.81±0.36 and 1.25±0.32 D, respectively; P<0.05). Refractive astigmatism decreased significantly in both group 1 and group 2, from 2.05±0.64 and 1.18±0.41 D preoperatively to 0.56±0.32 and 0.62±0.26 D at 3 months postoperatively, respectively; however, the magnitude of reduction was significantly greater in group 1 than in group 2 (P<0.05). Postoperative refractive astigmatism did not differ significantly between the two groups (P>0.05). In both groups, uncorrected and corrected visual acuity improved significantly after surgery (P<0.05), with no significant between-group difference (P>0.05).
  • Conclusion
    Combining an EDOF IOL with LRI may be effective for mild astigmatism, whereas selecting a toric EDOF IOL may better support visual improvement in patients with moderate to severe astigmatism.
현대 백내장 수술은 단지 백내장을 제거하고 인공수정체를 삽입하는 것으로 환자의 요구를 충족하기 어려워졌다. 최근에는 나이가 들어서도 근거리 및 중간거리를 안경 없이 맨눈으로 보고자 하는 환자의 수가 늘어나서 노안 인공수정체의 수요도 증가하였다.
다초점 인공수정체는 단초점 인공수정체와는 달리 잔여 난시에 대한 민감성이 높아서 중등도의 난시가 남았을 때 시력이 급격히 떨어지는 단점이 있다[1]. 난시로 인한 원거리시력 저하는 단초점 인공수정체보다 다초점 인공수정체에서 더 심했지만, 근거리시력은 다초점 인공수정체에서 더 좋았다[1]. 다초점 인공수정체를 삽입한 눈에 난시가 있는 경우 원거리시력이 전반적으로 저하되었으며, 이는 1.0 diopters (D) 이상의 난시를 교정할 필요가 있음을 시사한다[2].
백내장 수술에서 난시를 교정하는 것은 최대의 시력과 굴절 결과를 얻는 데 필수적인데, 백내장 수술을 받는 환자의 약 40%가 1.0 D 이상의 난시를 가지고 있다고 알려져 있다[3]. 백내장 수술 중 수술 전 각막난시를 줄이기 위해 다양한 방법이 사용된다. 본 연구에서는 중등도 난시가 있는 눈에서 두 가지 난시 교정 방법을 비교하였다. 백내장 수술 중 난시를 줄이기 위해 수십 년 동안 각막윤부이완절개술과 같은 저렴한 대안이 사용되어 왔지만[4,5], 이러한 방법은 상대적으로 예측하기 어렵고 완전한 교정을 위해 추가적인 보완 수술이 필요할 수 있다.
다초점 난시 인공수정체는 다초점 및 난시 광학 구성 요소의 조합을 통해 심각한 각막 난시가 있는 환자에게도 안경 의존도 감소라는 큰 이점을 제공한다. 다초점 난시 인공수정체를 통해 백내장 수술 중 주변부각막이완절개술 또는 윤부이완절개술[4,6], 투명각막절개술[7], 또는 굴절레이저수술[8]과 같은 추가 시술이 필요하지 않게 된다.
중등도의 각막난시에서 다초점 난시 인공수정체 삽입과 다초점 인공수정체 삽입 및 윤부이완절개술을 비교한 논문은 기존에 보고되었다[9]. 두 군 모두 굴절난시는 감소했지만, 다초점 난시 인공수정체 삽입군에서는 난시 인공수정체의 회전 안정성이 우수하여 수술 후 난시 교정 정도가 예측 가능했다[9]. 하지만 최근 논문에는 연속초점 인공수정체에서 난시 인공수정체와 윤부이완절개술을 비교한 보고는 없었다. 이에 저자들은 백내장 수술 시 연속초점 인공수정체에서 난시 인공수정체와 윤부이완절개술의 시력 개선 효과를 비교하였다.
수술 전 백내장으로 인한 시력 저하를 가진 환자들을 연구 대상으로 포함시켰고, 심한 각막 혼탁이나 불규칙한 각막 표면 등으로 굴절률이나 각막곡률을 측정할 수 없는 경우, 시력에 영향을 미칠 수 있는 기저 안과 질환이 있는 경우, 시력에 영향을 미칠 수 있는 특이 전신 질환이 있는 경우, 후낭파열 등의 수술 중 합병증이 발생한 경우는 연구 대상에서 제외하였다. 저자들은 후향적으로 차트 검토와 데이터 분석을 수행하였다. 이 연구는 기관 검토위원회의 규정 및 헬싱키 선언에 따라 수행되었으며 가톨릭대학교 부천성모병원 윤리위원회는 본 연구 계획서를 승인하였다(No. HC26RISI0033). 후향적 의무기록 분석 연구로 사전 동의는 면제되었다.
환자 검사
2019년 5월부터 2023년 11월까지 백내장 수술을 받은 환자 중 연속초점 난시 인공수정체(ZXT, TECNIS Symfony; Johnson & Johnson Vision)를 사용하여 수술을 시행한 60명(60안, 1군)의 환자군과 연속초점 인공수정체(ZXR00, TECNIS Symfony)와 윤부이완절개술을 함께 시행한 60명(60안, 2군)을 대상으로 하였다. 각막난시가 1.0 D를 초과한 경우 난시 인공수정체를 삽입하였고(1군), 각막난시가 1.0 D 이하인 경우 윤부이완절개술을 시행하였다(2군). 수술 전 검사로는 병력 청취, 나안시력 측정, 최대교정시력 측정, 현성굴절검사, 세극등 현미경검사, 안저검사, 안축장검사, 각막곡률측정검사 및 각막지형도검사(Pentacam, Oculus GmBH)를 시행하였다. 안축장은 IOLMaster 500 (Carl Zeiss Meditec Inc.)을 이용하여 측정하였고 측정되지 않는 경우 A-scan (Axis II, Quantel Medical)을 이용하였다. 1군의 경우 인터넷 기반의 계산프로그램(http://www.amoeasy.com/toric2)을 사용하여 인공수정체 종류와 도수를 선택하였다. 난시의 크기와 축은 Pentacam의 중심 3 mm 영역의 전체 각막난시를 이용하여 측정된 결과값을 대입하였고, 각막절개는 가파른 난시 경선에 시행하도록 계획하였다. 수술로 인하여 유발되는 각막난시는 0.5 D로 계산하였다.
현성굴절검사, 시력검사 및 세극등 현미경검사는 수술 후 1일, 1주일, 1개월, 2개월, 3개월에 시행하였다. 초점심도(defocusing curve)는 명소시(photopic condition, 100 cd/m2) 상태에서 한천석 시력표를 이용하여 측정하였다. 원거리 단안 시력을 완전히 교정한 상태에서 –4.5 D부터 +1.0 D까지, 0.5 D 단위의 구면렌즈를 교체하며 시력을 측정하였고, 그 결과값을 이용하여 초점심도를 얻었다.
수술 방법

난시 교정 인공수정체군

수술 직전 환자를 정위로 앉힌 상태에서 머리가 기울어지지 않았는지 확인한 후, 정면의 원거리 주시점을 바라본 상태로 toric reference corneal marker (AE-2793S, ASICO)를 이용하여 각막윤부의 3시, 6시 및 9시 방향에 기준표지자를 표시하였다. 구후 마취가 필요한 환자의 경우 마취 직전에 동일한 과정을 시행하였다. 수술 시작 시 환자가 누운 상태에서 기존에 표시한 표지자를 기준으로 하여 toric axis marker (K3-7910, Katena)를 이용하여 환자의 각막 위에 목표 난시축을 표시하였다. 모든 수술은 한 명의 숙련된 술자(ECK)가 시행하였다. 가파른 난시축의 방향에서 2.75 mm의 투명각막절개를 시행한 후 CENTURION (Alcon)을 사용하여 수정체유화술을 시행하였다. 인공수정체를 후낭에 삽입한 후 인공수정체 뒤에 남아 있는 점탄물질을 제거하고 인공수정체를 회전시켜 인공수정체의 축을 미리 표시한 위치에 평행하게 위치시켰다. 각막 봉합술은 시행하지 않고 수술을 종료하였으며, 수술 중 후낭 파열 등의 합병증이나 수술 후 합병증은 발생하지 않았다.

윤부이완절개술군

윤부이완절개술군의 경우 환자의 수술 전 데이터에서 확인한 각막난시 축을 기준으로 하여 난시가 가파른 축의 방향으로 중심각막 두께의 100% 깊이로 각막윤부에 6 mm 이완 절개를 시행하였다. 인터넷 기반의 계산프로그램(https://www.lricalculator.com)을 사용하여 난시교정의 양을 계산하였다. 그 후 난시 교정 인공수정체군과 동일한 방법으로 수정체유화술과 인공수정체 삽입술을 시행하였다.
통계 분석
통계 분석은 IBM SPSS ver. 22.0 (IBM Corp.)를 이용하였다. Wilcoxon signed-rank test를 사용하여 각 군의 수술 전후 시력과 굴절난시 및 각막난시를 비교했으며, 두 군 간의 비교는 Mann-Whitney U test를 사용하였다. P값이 0.05 미만인 경우 통계적으로 유의한 것으로 간주하였다.
수술 전 기록
수술 전 데이터를 비교한 결과, 평균 나이, 구면굴절대응치, 교정시력, 각막내피세포밀도(mm2)에는 두 군 간 통계적으로 유의한 차이가 없었다(P>0.05). 수술 전 각막난시(1군 1.65±0.56 D, 2군: 0.81±0.36 D), 굴절난시(2.05±0.64 D, 1.18±0.41 D), 전체각막난시(1.70±0.54 D, 1.25±0.32 D)는 모두 1군에서 2군보다 유의하게 높았다(P<0.05) (Table 1).
수술 후 난시
각막난시는 1군에서는 수술 전과 수술 3개월 후 사이에 통계적으로 유의한 변화가 없었으나(P>0.05), 2군에서는 수술 3개월 후의 각막난시(0.60±0.13 D)가 수술 전(0.81±0.36 D)에 비해 유의하게 감소하였다(P<0.05). 굴절난시는 1군과 2군에서 모두 수술 전(2.05±0.64 D, 1.18±0.41 D)보다 수술 3개월 후(0.56±0.32 D, 0.62±0.26 D)에 유의하게 감소하였으며, 1군의 감소 폭이 2군의 감소 폭보다 유의하게 컸다(P<0.05). 수술 3개월 후 굴절난시는 두 군 간 통계적으로 유의한 차이는 없었다(P>0.05). 전체각막난시는 1군에서는 수술 전과 수술 3개월 후 통계적으로 유의한 변화는 없었으나(P>0.05), 2군에서는 수술 3개월 후의 전체각막난시(0.61±0.28 D)가 수술 전(1.25±0.32 D)보다 유의하게 감소하였다(P<0.05) (Fig. 1).
수술 후 시력
수술 전과 수술 3개월 후 나안 원거리시력, 중간거리시력 및 근거리시력과 원거리 교정한 원거리시력, 중간거리시력 및 근거리시력은 모두 두 군 간 통계적으로 유의한 차이는 없었다(P>0.05). 1군과 2군에서 모두 수술 3개월 후의 나안시력(0.04±0.02 logMAR, 0.04±0.02 logMAR)과 최대교정시력(0.02±0.01 logMAR, 0.02±0.01 logMAR)이 수술 전의 나안시력(0.63±0.28 logMAR, 0.68±0.35 logMAR)과 최대교정시력(0.41±0.25 logMAR, 0.43±0.32 logMAR)보다 유의하게 개선되었다(P<0.05) (Fig. 2). 수술 3개월 후 각막내피세포밀도의 손실은 두 군 간 통계적으로 유의한 차이가 없었다(P>0.05) (Table 2).
탈초점 곡선
탈초점 곡선은 두 군 모두 탈초점 0 D (4 m)에서 최고점을 보였으며, 근시 탈초점과 원시 탈초점이 증가함에 따라 시력이 점차 감소하였다. 중간거리 영역(–1.5에서 –2.0 D; 50–67 cm에 해당)을 따라 부드러운 곡선을 보였고, –3.0 D의 탈초점 곡선(33 cm에 해당)에서는 시력이 급격히 떨어지는 결과를 보였다(Fig. 3).
일상 활동과 업무에서 중간거리 및 근거리시력의 사용이 증가함에 따라 노안 교정 인공수정체 삽입에 대한 환자들의 관심이 커지고 있다[10]. 다초점 인공수정체는 원거리시력을 유지하면서 근거리 및 중간거리시력을 향상시키는 데 탁월한 결과를 보여주었지만, 빛 번짐, 눈부심, 불편한 시야 등의 몇 가지 단점이 있다[11]. 최근 중간거리시력 향상을 위해 비구면 전면과 연속적인 도수 프로파일을 변형한 강화 단초점 인공수정체가 많이 사용되고 있다[12]. 강화 단초점 인공수정체는 단초점 인공수정체에 비해 우수한 중간거리시력을 보여주지만, 시력의 범위가 충분하지 않아 연속초점 인공수정체보다 근거리시력이 우수하지 않다[13]. 본 연구에 사용되었던 TECNIS Symfony는 회절 인공수정체로, 단일거리시력이 아닌 연속적인 시력의 범위를 제공하는 연속초점 인공수정체이다.
다초점 인공수정체를 삽입한 경우 단초점 인공수정체에 비해 잔여 난시로 인한 시력 감소가 더 크다. 특히 난시 인공수정체가 수술 후 회전을 일으켰을 때 잔여 난시가 변화하여 다초점 인공수정체의 시력이 더 떨어질 수 있다[14].
다초점 인공수정체 삽입 시의 난시 교정 방법으로는 각막절개술, 윤부이완절개술 및 난시 인공수정체 삽입이 있으며, 다초점 난시 인공수정체 삽입과 다초점 인공수정체 삽입 및 윤부이완절개술을 비교했을 때 굴절난시는 두 군 모두에서 감소했지만, 윤부이완절개술은 난시 교정 정도가 예측이 어려운 반면, 다초점 난시 인공수정체 삽입은 회전 안정성이 우수하여 굴절난시가 예측 가능하다는 보고가 있었다[9]. 단초점 인공수정체에서는 난시가 2.50 D 이상인 환자에게 난시 인공수정체 삽입술과 윤부이완절개술을 병행하는 수술이 효과적이라는 보고가 있었다[15]. 또한, 고도 난시 환자에서 난시 인공수정체 삽입술은 중도 난시 환자에서 난시 인공수정체 삽입술과 윤부이완절개술을 병행한 경우보다 특히 수술 후 초기에 더 나은 임상 결과를 보였다는 보고가 있었다[16]. 하지만 연속초점 인공수정체에서 난시 인공수정체와 윤부이완절개술에 대한 비교는 보고된 바가 없어 본 연구에서는 연속초점 인공수정체에서 난시 인공수정체와 윤부이완절개술의 차이를 비교하고자 하였다.
본 연구에서 각막난시와 전체각막난시는 1군에서 수술 전과 수술 3개월 후 사이에 통계적으로 유의한 변화가 없었으나(P>0.05), 2군에서는 수술 후가 수술 전에 비해 유의하게 감소하였다(P<0.05). 굴절난시는 1군과 2군에서 모두 수술 전(2.05±0.64 D, 1.18±0.41 D)보다 수술 3개월 후(0.56±0.32 D, 0.62±0.26 D)에 유의하게 감소하였으며, 1군의 감소 폭이 2군의 감소 폭보다 유의하게 컸다(P<0.05) (Fig. 1). 이는 이전의 다른 보고와 같이 난시 인공수정체의 난시 교정이 윤부이완절개술보다 더 효과적일 가능성을 시사한다.
난시의 교정 예측도는 윤부이완절개술의 경우 교정 예측이 어려웠으나 난시 인공수정체에서는 수술 전 예측 잔여 굴절난시는 0.15±0.11 D였고 수술 3개월 후 잔여 굴절난시는 0.56±0.32 D로 차이가 있었으나 통계적으로 유의하지 않았다(P=0.23) (Table 2). 그러므로 수술 전 난시의 양이 많더라도 수술 후 어느 정도 난시 교정량을 예측할 수 있어 난시 인공수정체는 효과적이라 말할 수 있다. 수술 전 예측치와 수술 후 굴절난시값이 달랐던 이유는 일정하다고 계산한 수술유발 각막난시의 양이 환자의 각막 경성도나 연령에 따라 달라지고, 수술 후 시간이 지남에 따라 인공수정체의 회전이 발생하기 때문으로 생각된다[17].
6 mm 윤부이완절개술을 선택한 이유는 Gills nomogram에 따라 결정하였다[18]. 탈초점 곡선은 이전의 연속초점 인공수정체의 보고와 같이 두 군 모두 탈초점 0 D (4 m)에서 최고점을 보였으며, 근시 탈초점과 원시 탈초점이 증가함에 따라 시력이 점차 감소하였다[19].
본 연구에서 수술 전과 수술 3개월 후의 나안 원거리시력, 중간거리시력 및 근거리시력과 원거리교정 원거리시력, 중간거리시력 및 근거리시력은 모두 두 군 간 통계적으로 유의한 차이는 없었다(P>0.05). 두 군 모두 수술 3개월 후 나안시력(1군: 0.04±0.02 logMAR, 2군: 0.04±0.02 logMAR)과 최대교정시력(0.02±0.01 logMAR, 0.02±0.01 logMAR)이 수술 전 나안시력(0.63±0.28 logMAR, 0.68±0.35 logMAR)과 최대교정시력(0.41±0.25 logMAR, 0.43±0.32 logMAR)보다 유의하게 개선되었다(P<0.05) (Fig. 2). 따라서 수술 전 1.0 D를 초과하는 각막난시를 가진 환자에서는 연속초점 난시 인공수정체를 삽입하는 것이 더 적절한 방법일 수 있으며, 1.0 D 이하의 각막난시를 가진 환자에서는 연속초점 인공수정체 삽입과 윤부이완절개술을 병행하는 것이 효과적인 방법일 수 있다.
탈초점 곡선은 두 군 모두 탈초점 0 D (4 m)에서 최고점을 보였으며, 중간 거리 영역(–1.5에서 –2.0 D; 50–67 cm에 해당)을 따라 부드러운 곡선을 보였고, –3.0 D의 탈초점 곡선(33 cm에 해당)에서는 시력이 급격히 떨어지는 결과를 보였다(Fig. 3).
본 연구에서의 제한점을 기술한다면 후향적 연구의 특성상 두 군의 비교에서 같은 각막난시를 가진 환자의 비교를 하지 못하였기 때문에, 군 간 결과 비교는 시술 방법 자체의 효과라기보다 초기 난시 차이에 영향을 받았을 가능성이 있다. 이로 인해 두 방법의 효과를 직접적으로 비교하기에는 일부 제한이 있을 수 있다. 또한 수술 후 난시 인공수정체의 회전 정도를 기록하였다면 더 정확한 비교가 되었을 것으로 생각된다. 그리고 수술 후 추적 기간이 3개월로 비교적 짧기 때문에 난시 인공수정체의 장기적인 회전 안정성과 윤부이완절개술의 지속성을 평가하기에는 제한이 있었을 것이다. 향후 연속초점 난시 인공수정체와 연속초점 인공수정체 및 윤부이완절개술 병행의 장기적인 효능을 비교하기 위해서는 수술 전 난시 정도가 유사한 환자를 대상으로 더 큰 표본과 더 긴 추적 기간을 적용한 무작위 다기관 임상시험이 필요할 것으로 판단된다.
결론적으로 연속초점 인공수정체에서 난시 인공수정체는 중등도 이상의 난시에서 굴절난시를 효과적으로 예측할 수 있는 범위에서 교정이 가능하였으며, 윤부이완절개술은 경도의 난시에서 각막난시를 어느 정도 교정하여 시력 개선 효과가 있었다. 그러므로 경도의 난시에는 연속초점 인공수정체와 윤부이완절개술의 병행이 효과적일 수 있으며, 중등도 이상의 난시에서는 연속초점 난시 인공수정체를 선택하는 것이 환자의 시력 개선에 도움이 될 수 있다.
